Volunteer of the Year Form

The Zesty Award for Outstanding Volunteerism (Volunteer of the Year) is to recognize an outstanding volunteer who has made a great impact on Food Forward’s work and mission in 2022. The volunteer will receive a $1,000 award and will be honored alongside our Fruitanthropist awardees later this spring.

WHO IS ELIGIBLE? THE VOLUNTEER MUST…

Be an active and unpaid volunteer with Food Forward
Not be a current or incoming Board Member
Have volunteered with Food Forward for at least 1 year
Agree to complete a W-9 to receive the $1,000 award once notified of their nomination

The winner will be notified WHEN and asked to complete this form to help us know a bit more about them.

In addition to your nomination, we will take other information (including number of events and hours they volunteered, length of time with Food Forward, etc.) into account as we evaluate nominees.
