This Release hereby grants Food Forward Inc. the irrevocable, perpetual, worldwide right and permission to take and use visual images and/or audio recordings of the Participant listed in any manner or media without notification including but not limited to use on Food Forward Inc. websites, publications, marketing materials, social media, and advertisements. Participant gives Food Forward Inc. consent to record, videotape, photograph, or otherwise capture Participant’s image, voice, and/or likeness.
Participant agrees that all images and recordings are owned by, and all rights are released to, Food Forward Inc.
Participant agrees that their participation is voluntary and grants permission to the rights of their image, likeness, and voice as recorded without payment or other consideration. Participant understands that visual images or audio recordings may be edited, copied, cropped, exhibited, distributed or otherwise altered at the discretion of Food Forward Inc. and waives all rights for approval or inspection of any visual images or audio recordings.
Participant agrees to release and forever discharge Food Forward Inc. and its affiliates, successors and assigns, officers, employees, representatives, partners, and agents, in their individual and/or corporate capacities, from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, and/or causes of action which Participant may have or claim to have against Food Forward Inc. in connection with this Release.
Participant hereby warrants that they are eighteen (18) years old or more and competent to contract in their own name or, if they are less than eighteen years old, that Participant’s parent or guardian has signed this release form below. This Release is binding on Participant and Participant’s heirs, assigns, and personal representatives.
Participant has read this Release before signing and freely accepts the terms.
